Michael Colotta is a Family Nurse Practitioner, board certified by the American Academy of Nurse Practitioners. He moved to the East Texas area several years ago where he obtained his Bachelor’s degree in nursing from Stephen F. Austin State University and his Master of Science in Nursing from the University of Texas at Tyler. His scope of practice encompasses health promotion, disease prevention, diagnostic and management of common and complex health care problems beginning in childhood and continuing throughout the aging process.

He began his career in healthcare by working in a busy orthopedic clinic when he was 17 years old. Since that time, he has logged over 20 years experience in various areas of patient care, including hospital medical/surgical, emergency room, interventional radiology, occupational health and rural health.

Michael has a unique combination of clinical education and experience and strives to provide the best health care possible. He believes in building long lasting and trusting relationships with patients and their families. He seeks to acquire a comprehensive evaluation of patients by taking the time to listen and understand their particular needs. His true desire is to assist the client in making informed, appropriate decisions regarding their health, as well as the health of their loved ones.

When Michael is not in his healthcare role he enjoys fishing, hunting and spending time with his wife Carey and their kids, Riley, Logan, Cayden and Lauren, ages 12, 10, 5 and 4 respectively.

Tad Titlow, NP

Tad Titlow is a Family Nurse Practitioner, board certified by the American Academy of Nurse Practitioners. He is a native to the East Texas area. He obtained his Bachelor’s degree in nursing from the University of Texas

and his Master of Science in Nursing from Texas Tech University at Lubbock, Texas. His scope of practice encompasses health promotion, disease prevention, diagnostic and management of common and complex health care problems beginning in childhood and continuing throughout the aging process.

He began his career in healthcare by working at East Texas Medical Center Emergency Department for 5 years then pursing his degree as a nurse practitioner.

Tad has worked in several settings as a nurse practitioner to include Emergency, Rural Health Center, and Private practice.

He strives to build a long trusting relationship with his patients and their families.

Tad seeks to acquire a comprehensive evaluation by listening and understanding their needs.

When Tad is not in his healthcare role he enjoys spending time with his wife Linda and their kids, hunting and horse riding.

His most important priority is his Savior, Jesus Christ.

Sally Thompson, FNP

Sally Thompson is a Family Nurse Practitioner, board certified by the American Academy of Nurse Practitioners. She was born and raised in Henderson.

She received her Bachelor’s of Science in Nursing from Texas Woman’s University and her Master from Texas A&M at Corpus Christi. She is excited to have the opportunity to provide health care in her hometown.

She has worked as an RN for 10 years in a variety of settings including inpatient medical, pediatrics and mother-baby units. She has also served as a public health nurse for Rusk County. As a nurse practitioner she has worked in an Asthma and Allergy specialty office and in Urgent Care settings. She is certified as a BioTE provider, DOT physical clinician, Certified by the American Academy Facial Esthetics for Botox and Juvederm and she is also an Asthma Educator.

Sally has a unique combination of clinical education and experience and strives to provide the best health care possible. She believes in building long lasting and trusting relationships with patients and their families.
