Allison Forehand Locke is a Family Nurse Practitioner dedicated to
providing compassionate, community-centered care. A graduate of the
esteemed Troy University, she earned her Master of Science in Nursing as
an Advanced Registered Nurse Practitioner with a specialty in Family
Practice in 2016. Allison is also a proud alumna of Florida State University,
where she received her Bachelor of Science in Nursing in 2005.
Allison’s journey in healthcare began at just 16 years old, volunteering at a
local hospital. Her lifelong calling to nursing was solidified when, as a
young adult, she survived a life-threatening bacterial infection in her heart.
That experience, coupled with her understanding that heart disease and
stroke take the lives of more women than all cancers combined, ignited her
mission to make a lasting difference in the health and well-being of her
community.
Throughout her career, Allison has been recognized for excellence in
leadership, including but not limited to:

A long-time resident of Bonifay, Florida, Allison has been married to her
husband, Jack, for over a decade. Together, they enjoy fishing, gardening, and all water sports. Deeply rooted in her community, Allison’s life and
career embody a simple but powerful philosophy: to heal, to educate, and
to serve with heart.
